Your school nurse is someone to get to know! They are essential to help assist your student with any health problem and should be seen as a friend and advocate.
Please know confidentiality is a professional responsibility that is strictly enforced. Professional registered nursing services are provided through the West Hartford Non-Public School Health Program. Our school nurses follow the guidelines and mandates put forth by the State of Connecticut and the West Hartford Non-Public School Health Program. If you have any questions or concerns please call or stop by to see the nurse in your school.

Notes From the Health Room - 2024
Required Health Forms Prior to the First Day of School
The following forms need to be printed and returned to the office no later than August 28th. You may also return the forms by emailing them to mailbox@stastmschool.org for Grades Pre-K to Grade 5 and tkradas@stastmschool.org for Grades 6 to 8.
Student Emergency Information Form (2024-2025)- (Yearly requirement for All students)
Early Childhood Health Assessment Record - (Yearly for Pre-K and entering Kindergarten.)
Health Assessment Record - Form must be less than one year old. Required for new students and prior to entry in grades 4 and 7, if not already submitted.
Authorization for the Administration of Medication - Form must accompany any medications brought to school. Medications must be brought to the nurse by an adult.
Parent permission for Acetaminophen/Ibuprofen Form (10+ years only) is Optional.
